Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA reviewed six children’s files and determined that two were incomplete, as the required regulatory forms were not completed. Although the forms were present, they were not filled out and missing signatures from the parent or authorized representative. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have parents/authorized representatives fill out the licesning forms and send Fresno Regional Office proof of completed forms by 12/22/25.
Based on record review, the licensee did not comply with the section cited above. Licensee assistant/daughter did not have completed certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee daughter/assistant will complete mandated reporter training and submit proof to Fresno Regional Office by POC due date.
