Based on observation and interview, the licensee did not comply with the section cited above.Licensee's CPR/First aid expired on December 2023. Licensee stated her and her assistant completed the training in December 2023 but cannot find proof of training to confirm that she has renewed training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will continue to look for her CPR/First aid certificate and if she cannot find it she will make another appointment to complete the training.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated she does not have 2 children's file and the other 4 children's files are also missing regulation forms which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have parents fill out the children's records forms and put it in their file. LPA to clear the deficiency with a visit.
