Based on interview and record review, the licensee did not comply with the section cited above. Licensee has not completed a fire drill since 01/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to conduct a fire drill and submit proof of correction to Fresno Child Care office by 01/23/2026.
Based on interview and record review, the licensee did not comply with the section cited above. The licensee stated that she has not yet renewed the certificate that expired on 07/28/25 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will renew her certificate and send a copy to Fresno Regional Office by 01/23/26.
Based on interview and record review, the licensee did not comply with the section cited above. The LPA reviewed six children’s files and found that three did not contain all required licensing forms which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated she will review all 13 enrolled children’s files to ensure that each file is complete with the necessary licensing forms by 01/23/26. LPA will conduct a proof of correction visit.
Based on record review, the licensee did not comply with the section cited above. Licensee did not have records of immunizations pertussis and measles. Staff #1 is missing pertussis. LPA Mercado informed Licensee immunization records need to be on file for herself and staff which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated will submit proof to Fresno Regional office of immunizations for her self and staff #1 by POC due date.
