Based on record review, the licensee did not comply with the section cited above in that the last documentated fire drill was in 2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to conduct fire drill logs once every six months and will conduct & document one within the next month and will email a copy of the fire drill log to LPA L. Habtom by POC date of 6/5/2023.
Based on record review, the licensee did not comply with the section cited above in that the assistant did not have proof of mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to have assistant complete mandated reporter training and will email a copy of the certificate to LPA L. Habtom by POC date of 6/5/2023.
Based on record review, the licensee did not comply with the section cited above in that the licensee did not have proof of the assistant's tuberculosis, pertussis & measles which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to provide proof of assistant's immunizations via email to LPA L. Habtom by POC date of 6/5/2023.
