Based on record review, the licensee did not comply with the section cited above in that the licensee did not document sleep checks and did not complete sleep plans for infants under 12 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email LPA L. Habtom a sample of sleep checks and the sleep plans by plan of correction date of 3/6/2025.

Based on observation, the licensee did not comply with the section cited above in that licensee was unable to unlock the safe for the weapons to confirm that the weapons and weapons and ammunition are locked and stored separately which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA will return to clear the citation.

Based on record review, the licensee did not comply with the section cited above in that one staff member's mandated reporter training certificate was expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email LPA copy of the certificate by POC date.

Based on observation and record review, the licensee did not comply with the section cited above when her first aid card expired last month which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall submit a current and valid first aid/CPR card to LPA by plan of correction date of 3/10/23. The course must be EMSA approved or through the American Heart Association or Red Cross.
