Type B citation - potential risk if not corrected

LPA met with Site Supervisor Virginia Becerra. LPA Garcia toured the facility inside and out and took a census. Currently there are three (3) outdoor play yards for this license. While LPA Garcia was inspecting the large play yard number two (2), LPA observed a play structure that was unserviceable due to six (6) large cracks located in the center of the slide. The largest crack observed, located in the center of the slide, measured approximately twenty (20) inches in length covered by packing tape. The other five cracks observed were approximately seven (7) inches in length located near the bottom of the slide. LPA Garcia also observed damage to the stairs leading up to the platform of the cracked slide. LPA observed exposed bare rusted metal due to the protective outer layer coating to be severely separating from the staircase. LPA also observed uneven terrain located in play yard two (2) creating tripping hazards to the children in care. LPA also observed two open storage sheds located on play yard two (2) and play yard three (3) that were open and were not locked or appear ho have been locked. In portable classroom C, LPA observed two (2) children’s toilets to have no toilet bolt cap covers . LPA observed sharp exposed rusted bolts on both toilets. LPA Garcia also observed rusty toilet valves in the same children’s bathroom. LPA also observed tall grass and weeds located in parts of all three (3) play yards where children in care have access to. Based on the information obtained by observation Virginia Becerra was advised that a citation shall be warranted.
