Based on observation, interview and record review, the licensee did not comply with the section cited above in one (1) out of three (3) staff members did not have a criminal record clearance or an exemption from the State Department of Social Services prior to employment, residence, or initial presence in the facility which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licencee will complete a Criminal Record Clearance and or Transfer form LIC9182 for the one staff and submit proof to Community Care Licensing by or before December 13, 2024.

Based on record review, the licensee did not comply with the section cited above when licensee failed to provide documentation that indicates checks have been conducted as required per infant safe sleep which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will document infant safe sleep checks and provide proof to LPA via photos by December 27, 2024.

Based on record review, the licensee did not comply with the section cited above in 1 out of her two staff had an expired Mandated Reporter Training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have her staff complete their Mandated Reporter Training and submit proof to LPA via Photo by December 13, 2024.
