Based on observation the licensee did not comply with the section cited above when LPA observed a broken child safety lock to an under sink cabinet located in the kitchen that contained a liquid detergent. Also obseved was disinfecting wipes assessable to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to replace the child safety lock and provide proof of the correction via photo to LPA by November 22, 2023

Based onobservation the licensee did not comply with the section cited above when LPA observed the following, crafting supplies, books, games, and paper goods on the kitchen table. In the kitchen, LPA observed that the counters were covered with kitchen items, (food products, glassware, and plastic kitchen items). LPA also observed the following in the children’s bathroom, spider webs, spiders, missing light bulbs, and the shower being utilized as a storage area. These observations pose a potential health and safety risk or personal rights violations to children in care

Resolution: Licensee agrees to correct the above deficiency’s and submit proof of the correction via photo to LPA by November 22, 2023

Based on observation LPA observed loose items (Plush blanket, toys other objects) in in one (1) play yard. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review safe sleep regulations and write a half page report on play pen requirements for children 0-2 years of age.

Based on interview and record review, the licensee did not comply with the section cited above. LPA observed that Licensee did not have 15 minute check logs for infants that were between the ages of 12 months and 24 months old which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will physically check on sleeping infants every 15 minutes and keep documentation on safe sleep log. Licensee will submit copy of safe sleep log showing two weeks of documentation to Fresno Community Care Licensing by 01/27/2023.

Based on observation the licensee did not comply with the section cited above. LPA observed the following: in the dining room, there were craft supplies, games, and books cluttered on the table and kitchen items (cups, plastic wear, food) that covered kitchen counter. In the family room, there were multiple toys, children's chairs that were dusty and/or had dark smudges. The glass doors in the family room also had fingerprints and dark smudges. A play rug in the family room also had many stains and was dirty. Children have acess to the above named areas. Licensee confirmed the observations today. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would wipe and clean the toys, wash the carpet, clean the glass windows, clear and clean her kitchen countertops and clean and declutter the kitchen table. LPA discussed the regulation above, reminding licensee that her home (including toys, furntinure and walls, windows) should be kept clean and orderly per the regulation above. Licensee will submit proof of correction by POC date 2/8/2022.

Based on interview and record review the licensee did not comply with the section cited above. Licensee had expired Mandated Reporter Training (10/2021) and assistant #1 did not have training completed. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she did not know that AB1207 Mandated Reporter Training was required every two years and did not know that assistant required the training. LPA reviewed the regulation above and encouraged licensee to review ccld.ca.gov website for guidance or reach our Department should she have any questions regarding requirements. Plan of Correction to be submitted by POC date : 2/8/2022.
