Based on interview and record review, the licensee did not comply with the section cited above in by not completing sleep log for one (1) infant in care past the age of 12 months which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she was told that sleep logs did not have to be kept for infants older than 12 months. LPA reminded Licensee of safe sleep regulations. Licensee stated she will preform sleep log check for all infants in care until 24 months and will submit proof to the Department for the week of 3/24-3/28/25 no later than 4/3/2025.
Based on interview, the licensee did not comply with the section cited above in not conducted disaster drills and documenting, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will conduct a disaster drill and document and send it to Licensing by 9/30/23 by email.
Based on interview and record review, the licensee did not comply with the section cited above in Licensee did not have a current Mandated reporter Certificate which expired on 6/11/23, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will retake the mandated reporter training and send Licensing a copy of the current certificate by email by 9/30/23.
