Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that all of the children in care were missing the signed/dated Parent's Rights form (LIC 995).
Resolution: Licensee agreed to begin storing the Parent's Rights forms at her facility and will send copies of the Parent's Rights form for all of the children in care to the Community Care Licensing Office by the Plan of Correction due date, June 13, 2025.
