Based on record review, the facility did not comply with the section cited above as an individual, who was not fingerprinted cleared or associated to the facility, was present and caring for children during today's visit which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will fingerprint the staff, and the staff will no return until the fingerprint is cleared and associated to the facility.

Based on record review, the licensee did not comply with the section cited above by not having CPR first aid certification, which poses an potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will call, schedule and attend CPR class. She will submit new card to CCL by 02/15/2023

Based on record review, the licensee did not comply with the section cited above as LPAs did not observed the immunization record of the children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to obtain the record of required immunization of each child and maintain in file. Licensee will send the documents to LPA.
