Based on interview and record review, the licensee did not comply with the section cited above. According to the licensee, she conducd the fire drill and earthquake drill on 6/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will practice with the childcare children by next week and email the log to the LPA.

The licensee was unable to find her mandated reporter training, the last mandated reporter training was expired on 7/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will complete her mandated reporter trining and email the certification to LPA by 2/13/2025
Based on record review, the licensee did not comply with the section cited above. LPA reviewed 4 children's files. Children's IZ need to update and many forms are missing information or not fill, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will review all the children's file and ask parent to update the Immunization record and fill out missing information
