Based on observation and interview], the licensee did not comply with the section cited above. During today's inspection, it was noted that the licensee has been absent from the facility since 10/10/2024 and is expected to return on 10/22/2024.which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee asked the assistants to contact all the parents to pick up the day care children. The facility will be closed until the licensee come back.

Based on record review, the licensee did not comply with the section cited above. The licensee's mandated reporter training is expired 6/5/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will completed her Mandated Reporter Training and email to LPA by 10/25/2024.

Based on interview and record review, the licensee did not comply with the section cited above. The assistants' CPR was completed on line and another assistant does not have the training yet. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will find the EMSA certify agency for her assistants to complete the training and email the certifications to the LPA.

Based on observation andinterview, the licensee did not comply with the section cited above.During today's inspection, it was noted that the licensee has been absent from the facility since 10/10/2024 and is expected to return on 10/22/2024. The licensee did not inform the Regional Office or notify the childcare parents about her absence. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA talked to the licensee on the phone, the licensee understood she will need to notify the Palmdale office and the parents in the future. In addition, the licensee will close the facility if she will be out the town.

Based on observation,interview, and record review, the licensee did not comply with the section cited above. LPA observed adult #1 present in the home without a fingerprint clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee is aware no adults shall reside or work in the home without a fingerprint clearance. Adult #1 is currently in process for a fingerprint clearance. Adult #1 shall not be present until the fingerprint clearance is granted by the Department of Justice.
