Based on observation, interview, record review the licensee did not comply with the section cited above, Licensee and Staff present at the facility on the time of inspection had expiered Pediatric CPR/First aid certificates, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they enrolled to a CPR class scheduled for February 7, 2026 and will submitt proof of completion to LPA via email by 2/9/26.
Based on record review, the licensee did not comply with the section cited above, Staff present at the time of the visit did not have the Pertussis vaccine, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, Staff will get the pertussis vaccine by 2/16/26 and email proof to LPA
Based on record review, the licensee did not comply with the section cited above, Licensee had 2 day care children present at her facility that have not received the required immunizations, per licensee she stated that she was unaware that this requirement can not be waived due to personal/religious reasons, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee she will call the guardian of the children and explain the regulations and no longer care for the children if proof of vaccines are not provided. If vaccines are provided Licensee will email proof to LPA. If guardian refuses to vaccinate, the children will be disenrolled.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. LPA observed 4 cribs and all have pillows and blanket inside the crib. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed all the pillows out the cribs.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. LPA asked for 4 children's files who attended the facility today. The licensee was unable to provide 3 children's file,which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed to email the children's paperwork by next Friday.
Based on observation, interview, record review, the licensee did not comply with the section cited above. LPA observed a lady (Ma. Elena Alvarez) does not have her lifescan and does not show on the facility assistant list, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The assistant left during the inspection. The licensee agreed Ma. Elena will not return to the facility until she is associate with children,
