Based on record review, the licensee did not comply with the section cited above. The licensee was unable to provide the fire drill and earthquake dill log, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed to practice fire drill and earthquake drill by 7/18/2025 and email the log to LPA
Based on interview and record review, the licensee did not comply with the section cited above. During today's inspection, the licensee was unable to find Assistants Immunization records for her staff,which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed to submit the assistants' shot record by 7/18/2025.
Based on interview and record review, the licensee did not comply with the section cited above. According to the licensee, she has 2 assistants. Assistant #1 is new, she did not have Mandated Reporter training. Assistant #2, the licensee was unable to find her information, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed to ask her assistants to complete their training and email LPA their Mandated Reporter Trainng certifications.
Based on record review, the licensee did not comply with the section cited above. LPA observed 5 children files. Only 1 child has all requirement paperwor (LIC 700, 995, 627 and IZ), which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed to email all missing forms to the LPA by July 18, 2025.

Based on observation and interview, the licensee did not comply with the section cited above. LPA called the licensee on 7/7/2025 to make sure she received the new requlation about pool safety. LPA was unable to talk to the licensee due to her phone numer is not correct. The licensee did not have The licensee did not have the new required equipment: Log, pool alarm, a rescue pole equipped with a body hook, with a minimum fixed length of 12 feet., and life ring, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee agreed that children will not go outside until she has all the pool equipments.
Based on the interview and record review, the licensee did not comply with the section cited above. During today's inspection, LPA observed in the Guardian that the licensee's son's fingerprint is still in process. The licensee stated that she received a letter stating her son is cleared; however, she was unable to provide the letter. LPA called the office for support. According to the licensee, her son had a ticket a few years ago, which caused a problem with his fingerprint process, posing an immediate health, safety, or personal rights risk to persons in care.
Resolution: The licensee agreed that her son will not return to the facility until she surrenders her license on August 1, 2025.

Based on interview and record review, the licensee did not comply with the section cited above. The licensee was unable to provided her assistant's MMR, DtaP and flu shot reocrd . which posesw/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. The licensee and her assistant are unable to find the CPR/First Aid training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation,interview and record review, the licensee did not comply with the section cited above. LPA observed assistant #1 does not have her fingerpring associate with the facility. In addition, the licensee's adult is also not fingerpring/associated with the facility, which poses an immediate health, safety or personal rights risk to persons in care. Guardian does not show their name.
Resolution: The assistant #1 left the facility around 2:00 pm. The licensee agreed the assistant #1 will not return to the facility until she completes the Livescan. For the licensee's son, he will leave the facility @ 7:00am and will not return after children leave the facility @ 6:00.
Based on observation, the licensee did not comply with the section cited above. LPA observed dog stools in the backyard near children play toys, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will clean the backyard before children go outside to play.

Resolution: Licensee has contacted University of Antelope Valley for a copy of her CPR/First Aid card. Licensee will send proof of completion to LPA Tamayo no later than 08/26/22.
