Resolution: Licensee will email copies of the signed LIC 9227 Individual Infant Sleep Plan for child #2 and #4 to LPA.

Based on record review, the facility did not comply with the section cited above by failing to provide the Infant Sleep Log for child #2 and #4 which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: The licensee will email copies of the Infant Sleep Log for child #2 and #4 to LPA.

Based on record review, the facility did not comply with the section cited above by failing to provide the licensee and assistant’s mandated reporter trainings which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: Licensee and assistant will email copies of their mandated reporter training certificates to LPA.

Based on record review, the licensee did not have a current CPR/First Aid training which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: Licensee will email a copy of her cardiopulmonary resuscitation/First Aid training certificate to LPA.

Based on record review, the licensee did not maintain copies of LIC 627 Consent for Emergency Medical Treatment for child #3, #4, and #5 which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: Licensee will email copies of LIC 627 Consent for Emergency Medical Treatment for child #3, #4 and #5 to LPA.

Based on observation and record review the licensee did not comply with the section cited LPAs were not able to observe Licensee's assistant immunization which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall provided proof of immunization for staff before staff is allowed to work at the facility.

Based on LPAs observation the licensee did not comply with the section cited above due to tools in rearyard are accessible to children, although licensee states the children are not playing in rear yard at this time due to weather, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit photos of tools remove for rearyard.

Based on observation and record review the licensee did not comply with the section cited above due to LPA's did not observe licensee's assistant California ID which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall provide a copy of licensee's assistant California ID

Based on observation and record review the licensee did not comply with the section cited above due to LPAs did not observe licensee's assistant signed Notice of Employee Rights which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall complete staff file and provide proof by signed documents to assigned LPA.

Based on observation and record review the licensee did not comply with the section cited above due to licensee mandated reporter expired on 06-29-21 and licensee assistant was not available for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and licensee's assistant shall complete the mandate reporter training by visiting mandatedreporterca.com web site

Based on LPAs(observation, the licensee did not comply with the section cited above due to cleaning solutions were accessible in bathroom used by children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the cleaning solution from children's bathroom during the inspection.
