Resolution: Licensee will provide copies of immunization records to LPA thru email.
Based on observation, the licensee did not comply with the section cited above, LPA's observed the left side of the outdoor play area containing wood, propane tank and AC unit accessible to children. On the right side there was debris, bricks and metal objects near the shed, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will clear the outdoor hazard areas, and make them inaccessible to children.
Based on observation, interview, record review, the licensee did not comply with the section cited above due to the mandated reporter training certificate expiring in 2021,which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will complete the Mandated reporter training and email a copy of the certificate to LPA.
