Based on observation and interview, the licensee did not comply with the section cited above. LPA observed a BBQ grill with Propane, outdoor pizza oven with propane. LPA also observed a outdoor storage needs a lock, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee asked her son to remove all the propane and BBQ grill.

Based on interview and record review, the licensee did not comply with the section cited above. LPA reviewed 5 children's files and 3 children does not have IZ records in the file. The licensee had to text the parents to text the IZ record. The licensee stated she is updating children's files. However, she was unable to provide the old IZ record. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will text all the parents to provide the immunization record and email to LPA by 10/15/2024.

Based on interview and record review, the licensee did not comply with the section cited above. A child was in the facility and did not have any enroll today. According to the licensee, the family was enrolled in the past and came back afther mother had new babe. The licensee was unable to find the folder, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will ask parents to fill out another enroll package and submit to the LPA by 10/15/2024 for review.

Based on observation and intervie), the licensee did not comply with the section cited above. The licensee has a hot spa in the backyard. The spa cover is on but it was unlock, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agree tol find the spa cover key and lock the cover.

Based on interview and record review, the licensee did not comply with the section cited above. The assistant does not complete the training. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agree to have her assistant complete the Mandated Reporter Training and email the certification to LPA

Based on observation and record review, the licensee did not comply with the section cited above. Licensee's First Aid/CPR expired on 06/25/21, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will retake her CPR/First aid training and send proof of completion no later than 01/18/23.

Based on observation, the licensee did not comply with the section cited above. LPA did not observe a carbon monoxide detector in the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof of purchase of carbon monoxide detector to LPA Tamayo no later than 12/21/22 and place in a prominent area in the home.
