Based on record review, the licensee did not comply with the section cited above by failing to maintain copies of LIC 627 Consent for Emergency Medical Treatment for child #3 which posed a potential Health, Safety or Personal Rights risk to persons in care.
Resolution: Licensee will email a copy of signed parents LIC 627 Consent for Emergency Medical Treatment for child #3 to LPA.
Based on record review, the licensee did not comply with the section cited above LPA did not obsever licensee's current mandated report training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will completed the mandated reporter traiining course and forward a copy of the certificate to assigned LPA.
