Per licensee, fire extinguisher was purchased sometime last year. LPA reviewed last report and observed that licensee did not provide proof during last annual inspection. This poses a potential safety rights risk to persons in care.

Resolution: Licensee will service fire extinguisher or provide proof of when it was purchased and submit it to LPA by plan of correction date.

Based on observation, the licensee did not comply with the section cited above as LPA observed 5 infant bouncers and 1 infant rocker stored (not in use) inside the detached garage (converted day care room) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During the inspection, Licensee immediately removed the prohibited items from the garage. LPA will conduct an unannounced follow-up inspection at the facility by 12/16/2022 to ensure that the prohibited items are permanently removed and no longer in use.
