Based on record review, the licensee did not comply with the section cited above in 1out of 3 adults present during today's inspection who were providing care. Adult 1 does not have a criminal record clearance which poses an immediate safety risk to persons in care.
Resolution: Licensee stated they will either have Adult 1 obtain a criminal record clearance or they will submit a declaration stating that Adult 1 will not provide care and will remain in off limit areas during hours of operation by 5/3/24. Proof of either item will be submitted to LPA via email.
Based on observation and interview, the licensee did not comply with the section cited above in 1 out of 1 fire extinguishers was not replaced of serviced since 2021 which poses a potential safety risk to persons in care.
Resolution: Licensee will provide proof of serviced fire extinguisher or purchase receipt for a new one.
Based on observation and interview, the Licensee did not comply with the section cited above by not having the fire extinguisher serviced annually, Licensee stated last serviced in 2019. LPA did not observe a service tag on fire extinguisher and arrow leaning towards red at time of inspection; this poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will provide a photo of service tag to LPA by POC date via email.
