Based on interview, the licensee did not comply with the section cited above. Licensee and her 2 helpers - Robert Waldrip & Nicholas Hart did not have a current Mandated Reporter Training certificate. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Hart stated that they will complete a Mandated Reporter Trng. online (mandatedreporterca.com). She will submit copies of certificate to the department no later than 2/20/23.
Based on record review, the licensee did not comply with the section cited above in 7 children's files reviewed today. Mrs. Hart did not transfer the immunization information onto the Blue Card. (FormCDPH 286). This poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided a supply of Form CDPH 286 to Mrs. Hart today. She stated that she will complete the information on the blue cards and submit copies to the department no later than 2/28/2023.
Based on interview, the licensee did not comply with the section cited above. Mrs. Hart and her two helpers - Nicholas Hart & Robert Waldrip did not have immunization record on file to show that they are immunized against Pertusis, Measles and influenza. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. Hart and her helpers will obtain and submit copies of immunization to the department no later than 2/28/2023.
