Based on observation and interview the licensee did not comply with the section cited above when one play structure containing swings was not bolted to the ground and poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will make the outdoor play structure safe for persons in care by bolting the structure into the ground by 11/07/2025. Licensee states moving forward will ensure all play structures are securely bolted to the ground.

Based on observation and interview the licensee did not comply with the section cited above when an off-limits garage containing clutter and hazardous items was made accessible to children due to unlocked door and also a screened trampoline accessible due to being unlatched with a broken zipper which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Deficiency for garage was cleared during visit. Licensee locked garage making inaceesible prior to the end of the inspection on 10/24/2025. Licensee states she will add a latch to the doorknob cover as a double measure to ensure inaccessibility moving forward. Licensee states she will remove trampoline from facility and provide proof of correction by due date of 10/27/2025.

Based on observation, interview and record review the licensee did not comply with evidence that 1/3 staff obtained a current tuberculosis test within the last year which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide a copy of a negative tuberculosis test for staff (1) by 10/31/2025. Licensee states moving forward she will ensure that all staff have a TB clearance within seven days after initial presence in the home.
