Based on observation, interview, record review, the licensee did not comply with the section cited above in that, licensee did not maintain Sleep Logs for infants 12-24 months of age, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will begin maintaining sleep logs for all infants 0-24 months of age going forward. Licensee stated that they will submit a copy of the sleep logs for all infants 0-24 months to LPA via email by due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in that, licensee did not obtain tuberculosis clearance for A1, A2, and A3, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will submit proof of tuberculosis clearance for A1, A2, and A3 via email to LPA by due date.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that, licensee did not maintain immunization record showing proof of pertussis and measles for A1, A2, A3, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will submit proof of pertussis and measles for A1, A2, and A3 to LPA via email by due date.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that, Adult 3 (A3) did not obtain criminal record clearance prior to residence and initial presence in the facility, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they thought A3's criminal record clearance was completed. A3 left the facility to obtain their clearance. Licensee was reminded that A3 needs clearance to return to the facility. Licensee stated that they would submit a plan of action to ensure that criminal record clearance is obtained and confirmed for all adults living and working in the home going forward. Licensee stated that the plan will be emailed to LPA by 2/24/2026.

Based on record review, the licensee did not comply with the section cited above in that, 5 out 5 children's immunizations were not documents on CDPH 286, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will complete CDPH 286 for all children and submit copies to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, licensee did not maintain emergency cards for children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send copies of the emergency cards to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, licensee did not have a current roster of children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will create a current roster and send a copy to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, LPAs observed animal feces throughout the backyard play area, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will clean the animal feces and monitor the backyard for cleanliness regularly. Licensee stated they will submit a video of the clean backyard to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, licensee did not have LIC 9227 for infant under 12 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will complete the LIC 9927 for Child 1 and submit copy to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, licensee did not complete an emergency drill within the past six months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will conduct an emergency drill and submit a copy of the drill log to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, licensee did not maintain a 15 minute check log for infants under 24 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit copies of the 15 minute check logs to LPA via email by due date.

Based on record review, the licensee did not comply with the section cited above in that, 2 out 5 children did not have an immunization record, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain immunization records for Child 2 and Child 3, then submit copies to LPA via email by due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed 5 out 7 children were missing immunization records and licensee could not provide LPA with the records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit immunization records for five children to LPA by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file review, LPA observed 2 out of 7 children were missing LIC 995A Notification of Parent Rights which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit proof to LPA showing parents received LIC 995A Notification of Parent Rights by POC due date.

Based on observation and interview, the licensee did not comply with the section cited above. During children file review, LPA observed child #1 (C1) did not have documentation showing licensee was documenting infant safe sleep and licensee was unable to provide proof showing the 15 minute checks during nap times which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof showing she is documenting infant safe sleep for all infants under the age of 2 years old by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During staff file review, licensee was unable to provide a file for staff#2 (S2) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to submit a complete file for S2 showing LIC 9052 Employee Rights, Immunization of measels, pertusis, and influenza (or documentation of exemption), TB clearance, LIC 9108 Statement Acknowledging to Report Child Abuse and Mandated Reporter AB1207 certification by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During staff file reviews, LPA observed licensee and two assistants did not have a current mandated reporter certification. Licensee mandated reporter certification expired 6/25/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit documention to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above. While touring the outside area of the home at 9:51 a.m. LPA observed dog feces near the childrens play structure and toys which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed the dog feces picked up during the inspection.

Based on observation, the licensee did not comply with the section cited above. Upon arrival at 9:45 AM, LPA observed the stairs leading to the second level of the family child care home was not fenced or barricaded. LPA observed a child safety gate but the gate was opened and not locked giving children in care access to the second level of the home which is off limits which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed the gate leading to the second level of the home being closed during the inspection.
