Based on observation, interview, and record review, the licensee did not comply with the section cited above in. Licensee confirmed she did not conduct a disater drill within the past 6 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will conduct fire/disaster drills and will document. A statement needs to be sent to Licensing office stating will comply with fire drill requirements. Licensee will provide a copy of drill with plan, time and date, and emailed, mailed or faxed by the due date.

Based on observation, interview,and record review, the licensee did not comply with the section cited above. The licensee was unable to show proof of immunization/immunity against measles for one of her emlployee, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee made an appointment to obtain the required immunization/proof of immunity against measles during LPA's inspection. The licensee will send a copy of the immunization record to our office by the due date of 3/29/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 infant (o-24months) does not have a prove and doucuments for 15 mins infant safe sleep, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, After today March 12, 2024 she will doucemnts the infant safe sleep and email to LPA by the due date. LPA also provided the example of infant safe sleep to Licensee.
