Based on interview and record review, the licensee did not comply with the section cited above in that, licensee's CPR & First Aid certification was expired, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they have planned to complete their CPR & First Aid training in August. Licensee stated they will submit proof of their renewed certification to LPA via email by due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above .At 9:20 am during staff file review licensee’s mandated reporter training certificate was expired on 8/5/2021 and out of compliance. which posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensees stated would complete the mandated reporter training and submit proof of completion to licensing by the due date.
