Based on observation, interview, record review, At 11:00am, during staff file review Staff #2 (S2) immunization records were not available for review. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they did request copies of staff's immunization records and staff did provide a copy but they were misplaced. Licensee will request for copies again and will send them to licensing department by the due date.
