Based on observation and record review, the licensee did not comply with the section cited above in one count out of one file, Adult # 1 missing the current CPR and 1st aid which poses a potential health, and safety risk to persons in care. Adult # 1's Pediatric CPR/First Aid certifications expired on 9/17/2024 and the licensee has enrolled in part of the CPR and 1st aid training on 5/12/2025 and has not obtained a current/final certificate yet.

Resolution: The licensee agreed to send Adult # 1's proof of this document to LPA's email address by 5/20/2025.

Based on observation, interview, and record review)], the licensee did not comply with the section cited above. LPA observed licensee First Aid/ CPR certification expired on March 7, 2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to enroll in a First Aid/CPR course and provide LPA documentation of the certification by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file review, licensee was only able to provide 2 out of 5 children files to LPA which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have the children files completed by POC due date and send proof to LPA

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide LPA proof that she is documenting infant safe sleep. Licensee stated she did not have a safe sleep log. in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided licensee a safe sleep log. Licensee agreed to document infant safe sleep and provide LPA a copy of the documentation by POC due date.
