Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not have immunization record for C1, C2, and C5 for LPA's review, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee sent a text message to C1's parent during the visit. However, C1's parent had issues with the website to obtain the immunization record. Licensee was able to obtain immunization record for C2 and C5 during the visit. Licensee stated that they will email a copy of C1's immunization record to LPA by due date.
Based on interview and record review, the licensee did not comply with the section cited above in that, licensee stated that they have not updated their facility roster, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will update their faciity roster and email it to LPA by due date.
