Based on record review, the licensee did not comply with the section cited above in the licensee's assistant did not have a copy of the immunization record in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of the asisstant's immunization record to LPA via email by the POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide LPA with a fire/disaster drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. While touring the home, LPA observed four (4) infants inside a bedroom with the door closed placed inside crib which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee open the door during the visit.

Resolution: Licensee agreed to submit proof showing all infants in care have an LIC 9227 Individual Infant Sleeping plan form inside their file.
Based on observation and interview, the licensee did not comply with the section cited above. Licensee was unable to provide documentation of an infant safe sleep log showing infants are being checked every 15 minutes during nap time which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to document infant safe sleep every 15 minutes and submit prooof to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above. At 11:26 AM, LPA observed assistant (S1) starting to change childrens diaper on the changing table located inside the living room area. LPA observed assistant change five childrens diapers. Out of the five diapers changed, three were soiled. LPA also observed assisant using the same gloves in between diaper changes and not sanitizing the changing mat after each child which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a diaper changing policy and procedures to LPA by POC Due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed 2 out of 8 children present did not have a file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to send proof of file to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above. Upon arrival at 9:25 AM LPA observed assistant caring for eight (8) children. Out of the eight (8) children, five (5) were infants which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee arrived at the home at approximately 10:05 AM.
