PAIZ FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in that LPA observed 2 plants with thorns in the backyard, along with razors in an upper bathroom cabinet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will remove plants from the backyard. Licensee will place lock on cabinet in bathroom. Licensee will send photos of corrections to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/21/2025.

Based on observation and interview, the licensee did not comply with the section cited above in that medicine, poisons, and knives are stored in an upper kitchen cabinet with no lock which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will place a key lock on the kitchen cabinet to ensure it is inaccessible to all children enrolled. Licensee will send photos of corrections to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/21/2025.

Based on observation, licensee did not comply with the section cited above in that C4 was sleeping with a blanket which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will review infant safe sleep regulations and send email confirming they have read and understand regulations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/21/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that S2 did not have complete immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send copy of S2’s complete immunization record to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/21/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that C3 and C4 did not have commplete immunization record on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit complete immunization record for C3 and C4 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/21/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have sleep logs available to review for C4 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send a weeks’ worth of 15 minute sleep documentation of all infants in care to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/21/2025.
