Based on interview and record review, the licensee did not comply with the section cited above by not ensuring to document their disaster drill which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a new drill, document the drill, and send a copy to LPA.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing LIC 282 for C1 and C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure the receive the completed LIC 282 for C1 and C2 and send proof to LPA.

Based on observation and record review, the licensee did not comply with the section cited above in having an expired mandated reporter certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete their mandated reporter training and send proof of the completed course to LPA.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing sleep logs for C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete sleep logs for C1 and send LPA a copy of the completed logs.

Based on record review, the licensee did not comply with the section cited above in 3 out of 3 children missing LIC282 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will obtain LIC282 from all families and submit proof of completion via email to LPA.

Based on record review, the licensee did not comply with the section cited above as three children were missing documentation of sleep checks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will begin documenting sleep checks for children until age 2 years and provide proof to LPA.

Based on interview and record's review, the licensee did not comply with the section cited above in and has not completed the mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide mandated reporter training certificate training to the Department by due date 04/04/2023.

Based on observation and interview, the licensee did not comply with the section cited above due to there being ammunition and firearm's not being stored and locked seperately in the home in a area that was off limits to the children. Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to store firearms and ammunition per Title 22 regulations, and will send plan and proof to the Deparment by due date 04/04/2023.

Based on observation, the licensee did not comply with the section cited above in that the outdoor play structure is not in good repair, and has a crack on the bottom of the slide, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will bring the play structure to good repair and will send proof of correction to the Department by 04/21/2023.

Based on interview and record review, the licensee did not comply with the section cited above in four out of four records reviewed (C1-C4.) Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update childrens records and add completed LIC 282's to the children files, will provide proof of training to the Department by due date by 04/04/2023.
