Based on observation and interview, the licensee did not comply with the section cited above in he outside playground was full of cobwebs and debris, photos were taken, director stated the playground is cleaned daily. This poses a potential health, safety or personal rights risk to persons in care.

Director stated the rooms are cleaned at end of day each day once the children are gone. which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interviews and record review, school age children are being combined in the first and last hour of the day, with children as young as 13 months with only one teacher and no aide. Due to school agers being commingled ratios are being exceeded.
