Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews at 11:55 AM, LPA observed child # 4 (C4) and child #6 (C6) did not have immunization records in their file. Licensee was unable to provide proof of documentation during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send immunization records to LPA for C4 and C6 by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. At 9:20 AM LPA observed licensee and assistant caring for 13 children which included 1 infant and 12 preschool age children playing in the backyard. None of the children were 6 years old or enrolled in public school. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated today is one of the children's last day at the facility.
Based on record review, the licensee did not comply with the section cited above. LPA observed licensee has not conducted a disaster/fire drill since July 2023. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will conduct a fire/disaster drill and will send a copy of the drill log to LPA by POC due date.
Based on record review, the licensee did not comply with the section cited above. LPA did not observe a mandated reporter training certificate in licensee's or assistant's file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she and her assistant will complete the mandated reporter training and will send a copy of the certificate to LPA by POC due date.
