Based on observation, interview, record review, the licensee did not comply with the section cited above. When entering the home at 10:15 AM, LPA observed licensee caring for six children. LPA observed 4 out of 6 children were infants. Licensee stated there were no other children or adults in the home. which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: During the visit, LPA observed Child (C4) being picked up so that the licensee was back in ratio.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee mandated reporter expired 11/3/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit proof of certification to LPA by POC due date.
Based on observation,and record review, the licensee did not comply with the section cited above. During children file review, LPA observed children in care were missing LIC 700 Identification and Emergency Information which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit proof of childrens LIC 700 Identification and Emergency Information to LPA by POC due date.
