Based on observation, the licensee did not comply with the section cited above. While touring the back yard, LPA observed an open gate leading to the side of the home. LPA observed two propane tanks, along with Hot Shot insect killer spray, Spectracide spray, Ortho Pesticide, and other hazardous items inside an unlock storage cabinet which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee close the side gate during visit.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated she has not conducted or documented disaster/fire drill since 2020 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will conduct and document a fire/disaster drill by POC due date and submit documention to LPA via email at dean.thompson@dss.ca.gov
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee mandated reporter certification expired 11/29/20 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will attend the training and provide proof of certification to LPA via email at dean.thompson@dss.ca.gov
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide the individual sleeping plan LIC 9227 for child (C1) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: An Individual Infant Sleeping Plan LIC 9227 document was provided. Licensee stated she will have the form filled out by POC due date and submit the document to LPA via email at dean.thompson@dss.ca.gov
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated she checks on child (C1) but couldnt provide proof she is documenting infant safe sleep every 15 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided licensee with an infant safe sleep log and licensee stated she will submit proof showing she is documenting infant safe sleep every 15 minutes to LPA by POC due date to dean.thompson@dss.ca.gov
