During children file reviews at 9:50 AM, LPA observed Child (C1) did not have form LIC 9227 Individual Infant Sleeping Plan inside file. Licensee stated she was unaware of the form which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit proof to LPA via text, email, or mail by Plan of Correction due date that shows C1 LIC 9227 form has been filled out and complete.
