Based on observation and interview, the licensee did not comply with the section cited above in that C1 was sleeping in a recalled rocker (Fisher-Price Infant-to-Toddler and Newborn-to-Toddler Rockers) which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will toss out the recalled rocker when it is trash day. LPA observed the licensee move the rocker to the garage until trash day.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that the Licensee did not have any documentation to show that they have the infant safe sleep every 15 minutes, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will email LPA a copy of the 15 minute safe sleep check by due date.
Based on record review, the licensee did not comply with the section cited above in two counts out of 2 staffs' files lacking verification of MMR and Pertussis Vaccines which poses a potential health, safety risk to persons in care. Staff # 1 and staff # 2 did not have immunization on file.
Resolution: The facility representative agreed to send the verification of immunization for staff # 1 and # 2 to LPA's email address by the due date of 7/17/2024.
