Based on observation, the licensee did not comply with the section cited above in having a serviced or purchased fire extinguisher within the last year which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, she will purchase a new fire extinguisher and provide prood of purchase by POC due date of 08/21/25.
Based on record review, the licensee did not comply with the section cited above, Licensee Assistant (S #2) did not have mandated reporter training in the last two years which posesa potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, S #2 will take mandated reporter training and will provide documentation/proof of certificate to LPA by POC due date of 08/21/25
Based on observation, the licensee did not comply with the section cited above as the fireplace does not have a screen or barricade preventing access which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase and install a metal screen attached with a hook and will submit a photo via email of installed metal screen.
