Based on observation and interview, the licensee did not comply with the section cited above in, laundry room had deteregents, and cleaners, licensee stated the infant sleeps in the laundry room with door opent. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove the play yard from the laundry room-place a child safety latch to the master bedroom and laundry room to make the rooms inacessible to chidlren in care and send photos to LPA Flores via email by 8/06/24.

Based on observation, the licensee did not comply with the section cited above in, play yard is in between the laundry room door and the garage door, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will not utilize laundry room for infant to sleep in the future, licensee will send photo to LPA Flores via email of the laundry room being made inaccessible to children in care by 8/06/24.

Based on record review, the licensee did not comply with the section cited above, the assistant does not have immunization records on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will gather records for assistant and send to LPA Flores via email by 8/06/24.

Based on observation and interview, the licensee did not comply with the section cited above, the infant play yard was in the laundry room, licensee stated the infant sleeps in the room due to noise and the door remains open. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove the play yard from the laundry room and send LPA Flores photo via email by 07/24/24.

Based on record review, the licensee did not comply with the section cited above in infant does not have sleep log on file at the time of inspection. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee sent home the children at the time of inspection-licensee will maintain a sleep log beginning today 7/23/24 and send to LPA Flores via email a week of infant safe sleep log for infants in care via email.

Based on interview and record review, the licensee did not comply with the section cited above in that licensee and two assistants did not have a current madated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreees to complete training and submit proof to the Department by due date 02/03/2023
