Based on interview & observation the licensee did not comply with the section cited above in Bedroom where child #1 sleeps has vicks and prescription medications on the shelf of the bed, accessible to chidlren in care.which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee informed LPA's that child #1 sleeps in the swing and sometimes will be placed on the bed with her adult daughter (Staff #3). licensee will have items removed and place child #1 in play yard when sleeping.
Based on observation, the licensee did not comply with the section cited above in Bathroom drawer to the left of the sink had prescription medications, toothpaste and troothbrushes accessible to children in care. this posed a potential health, safety or personal rights risk to persons in care.
Resolution: Staff #2 fixed the latch on the drawer at time of inspection.
Based on observation, the licensee did not comply with the section cited above in play yard in master bedroom is located directly under a mounted guitar on the ceiling. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was asked to remove the guitar or place the play yard in a different location in the master bedroom to prevent risk to children in care.
Based on observation and interview, the licensee did not comply with the sectioin cited above, child #1 sleeps in a swing, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that Child #1 sleeps in the swing and is sometimes removed to lay down with adult daughter on the bed. Licensee was informed that Child #1 needs to have own play yard due to age and cannot be left in the swing for sleep. Licensee will have child#1 removed immediately from swing once C1 has fallen asleep.
Based on record review, the licensee did not comply with the section cited above staff #2, staff #3 did not have current mandated reporter on file at time of inspection, This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send Mandated reporter certificate to LPA Flores via email by 11/09/23
Based on interview & record review, the licensee did not comply with the section cited above in licensee's CPR/First Aide expired on 9/25/23, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee showed LPA's at time of inspection she has completed first half of CPR/First Aide, however will be completing the second half this weekend. Licensee will send to LPA Flores via email a copy of updated CPR/First aide by 11/09/23.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in documenting the sleep log for the 3 infants in care during the visit which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to create a sleep log for the infants (C2, C9, C10) and submit a copy of the completed log to the department by the POC due date.
Based on record review, the licensee did not comply with the section cited above in renewing her and her assistant's Mandated Reporter Trainings which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the Mandated Reporter Trainings and submit a copy of the certificates to the department.
Based on record review, the licensee did not comply with the section cited above in having LIC 9227 in the files of the 3 infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete LIC 9227 for the 3 infants (C2, C9, C10) in care and submit it to the department by the POC due date.
