Based on interview and record review, the licensee did not comply with the section cited above in child 1 did not have a sleep log at time of inspetion. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee did not have a previous sleep log for any infant in care for the past. Licensee informed LPA that she throws them away at the end of the week. LPA informed Licensee all forms are required to be kept in the child's file for three years. Licensee will maintain a sleep log for all infants in care 24 months and under and send LPA Flores, copy of sleep log for the next two weeks via email by 6/11/24.
Based on record review, the licensee did not comply with the section cited above in, licensee;s spouse did not have current Mandated Reporter on file at the time of inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain current Mandated Reporter Certificate AB1207 and submit to LPA Flores via email by 6/11/24.
Based on observation & interview, the licensee did not comply with the section cited above the bedroom door infants were sleeping was closed. This posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee has a camera monitor, checks on the chidlren every 15 minutes. LIcensee was informed door cannot be shut for any chidlren napping, Licensee immediately opened the door and said she will keep it open from now on
