Based on record review, the licensee did not comply with the section cited above in 3 out of 4 staff do not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain a copy of Staff 2-4 immunizations by POC date (08/30/2024) and submit proof to LPA.
Based on record review and interview, the licensee did not comply with the section cited above in licensee was unable to provide proof of the 15 minute log and stated that she was unaware that the 15 minute log needed to be completed when staff are near the infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will start the sleeping log for infants by POC date 08/08/2024 and submit proof to LPA.
Based on record review, the licensee did not comply with the section cited above in the licensee does not have a current Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete trianing by POC date (08/30/2024) and submit proof to LPA.
Based on observation, the licensee did not comply with the section cited above in the licensee had an above ground swimming pool that did not have a gate/fence that met the above regulation which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee emptied out the swimming pool prior to LPA leaving the facility.
Based on record review, the licensee did not comply with the section cited above in the licensee did not have a signed LIC9227 for Child #2 (C2) -Child #4 (C4) which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that the parents of C2-C4 sign the LIC9227
Based on observation, the licensee did not comply with the section cited above in Child #2 was napping and had an attachment on her pacifier along with a bib which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the pacifier attachment and bib. LPA reminded licensee that there should be nothing attached to the pacifier or around the infants neck while napping.
Based on observation, the licensee did not comply with the section cited above in Child #4 was napping and there was a blanet hanging over the corner of the crib which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the blanket and LPA reminded licensee that there should be nothing hanging over the crib or any loose objects.
Based on observation and record review, the licensee did not comply with the section cited above in that 4 out 4 staff did not have proof of completion of the Mandated Reporter training (AB 1207) on file which poses a potential health, safety and personal rights risk to persons in care.
Resolution: Copies of the Mandated Reporter certificates of completion will be provided to LPA.
Based on observation, the licensee did not comply with the section cited above in that 2 out of four infants were observed to be using pacifiers that were attached to pacifier clips attached to the children's clothing, which poses a potential health, safety and personal rights risk to persons in care.
Resolution: Licensee's husband, Manuel Maldonado stated the parents of the children will be informed that no pacifier clips are allowed. Pacifier clips will be removed from the facility immediately.
