Resolution: Per licensee has ordered a new carbon monoide detector and is due to arrive by 01/09/2026. LPA will conduct an inspection to vew and test the carbon monoxide detector by POC due date.

Resolution: Per licensee, assistant has complete mandated reporter training but does not have access to view. Per licensee will request it from assistant and place it in the file by POC due date.
