Based on interview and record review, the licensee did not comply with the section cited above in that 15 minute sleep checks for Child #1 were not documented and available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will submit documentatin of 15 sleep checks for Child #1 beginning 5/14/2024 - 5/17/2024 and submit proof to the San Diego Regional Office, no later than 5/20/2024.
Based on interview and record review, the licensee did not comply with the section cited above in that licensee and assistant #1 did not have mandated reporter certification available for review during time on inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will submit mandated reporter certification for herself and assistant #1 to the San Diego Regional Office, no later than 5/27/2024.
