Based on interview and record review, the licensee did not comply with the section cited above in that C1 and C2 who are infants did not have 15 minute sleep checks documented and available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will document and provide proof of 15 minute sleep checks for C1 and C2 for the period 8/15/24 - 8/22/24 and provide proof to the San Diego Regional Office, no later than 8/23/24.

Based on observation, interview, record review, the licensee did not comply with the section cited above in which poses a potential health and safety risk to persons in care. The Licensee and the adult helper did not renew their online mandated reporter certification (AB1207). Their certification expired on 02/02/2022.

Resolution: The Licensee stated she and the adult helper will renew their online mandated reporter certification (AB1207) by September 30, 2022 and will submit proof of correction to LPA by 09/30/2022. The Licensee will ensure that the mandated reporter certification (AB1207) is renewed every two years prior to expiration date.
