Based on observation the licensee did not comply with the section cited above by not ensuring the qualified teacher present was supervising no more than 12 infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation and record review, the licensee did not comply with the section cited above as one, out of nine children, does not have immunization records as required by Title 22 which poses a potential health, and safety risk to persons in care. Child #8 is does not have the immunization records on file during the inspection.

Based on LPA's observation and record review, the licensee did not comply with the section cited above three out of the nine children's records reviewed were missing the medical assessments with TB results for child #1, #2, #4 and child #9 was missing the TB results which poses a potential health, and safety risk to persons in care.

Based on record review, the licensee did not comply with the section cited above for 4 out of 8 staff which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in that S1 did not have immunizations records available for review for measles which poses a potential health, safety or personal rights risk to persons in care.
