Resolution: Licensee stated that he will submit documentation of fire/disaster drill and a video of the drill being conducted to the SDRO as proof, no later than 2/27/2024.
Based on interview and record review, the licensee did not comply with the section cited above in 7 out of 7 persons as children records for 7 daycare children did not have Identifcation and Emergency Information (form LIC 700) or Consent for Medical Treatment (form LIC 627) available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that he will submit proof of form LIC 700 for 7 daycare children to the SDRO, no later than 2/28/2024.
Based on interview and record review, the licensee did not comply with the section cited above in 7 out of 7 persons as children records for 7 daycare children did not have Affidavit Regarding Liability Insurance (form LIC 282) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that he will submit proof of form LIC 282 for 7 daycare children to the SDRO, no later than 2/28/2024.
