Based on observation and interview, the licensee did not comply with the section cited above in 7 out of 7 persons as LPA observed shampoos, razors, body wash, and facial wash in the shower of the hallway bathroom and perfume in bedroom #1, which pose a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee removed the shampoos and razors and secured the shower with a safety strap lock during the time of inspection. Licensee locked the door to bedroom #1 where the perfume was located.
Based on interview and record review, the licensee did not comply with the section cited above in 7 out of 7 persons, fire/disaster drill documentation was not available for review during time of inspection. Licensee stated that there has not been a fire/disaster drill conducted at the facility, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will conduct a fire/disaster drill with the daycare children and document the drill. Licensee will provide LPA with documentation no later than, 8/18/2023.
Based on observation and interview, the licensee did not comply with the section cited above in 3 out of 7 persons, as LPA observed three children in bedrooms #1, #2 and #4 with the doors closed, C1 and C2 are infants, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the children from the bedrooms and locked the doors during the time of inspection.
Based on interview and record review, the licensee did not comply with the section cited above in 7 out of 7 persons as mandated reporter training for licensee expired on 6/30/2023 and was not available for licensee's assistant, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant provided proof of mandated reporter training scheduled for 8/21/2023. Licensee stated that she will provide proof of completed training no later than, 8/28/2023.
