Based on the information obtained during investigation interviews, observations, and pictures received is determined that child C1 was injured due to facility not providing adequate supervision on 3 incidents which poses/posed an immediate health, safety or personal rights risk to children in care.

Based on record review and interview, the licensee did not comply with the section cited above in 7 out of 10 staff members which poses a potential health, safety or personal rights risk to persons in care.
Care and Supervision. No child(ren) shall be left without the supervision, including visual observation This was not met as evidenced by..... On 2/27/25 Child (C1) wandered away from the drop off area. which poses a potential health, safety and personal rights risk to children in care. child managed to open the chain linked fence, pass through a gated area, descend a flight of stairs, to reach and push open the security fence. Staff member (S1) intercepted the child at the gate door below, preventing any harm from occurring. This poses a potential health, safety and personal rights risk to children in care.
Based on investigation interviews, staff S1 used inappropriate discipline practices by withholding food from children. Although food was never withheld from a student for an entire meal, a student may need to wait until all the other children were finished or have it delay as a form of disciple which poses/posed an immediate health, safety or personal rights risk to children in care.
